Canine pyometra is a severe reproductive tract disease associated with prolonged progesterone exposure during diestrus, endometrial alterations, impaired local immune defence, and bacterial colonisation of the uterus. This review synthesised current evidence on the pathogenesis of canine pyometra, patient selection for conservative management, and the pharmacological basis of treatment. d the pharmacological basis of treatment. A systematic literature review was conducted using PubMed, Scopus, Web of Science, ScienceDirect, and Google Scholar. Search terms addressed canine pyometra, conservative treatment, aglepristone, prostaglandins, antiprogestins, and reproductive outcomes. Clinical and experimental studies, systematic reviews, and clinical recommendations were included. Owing to substantial clinical and methodological heterogeneity among the available studies, quantitative metaanalysis was not undertaken. The available evidence indicates that prolonged progesterone exposure promotes secretory and cystic endometrial changes, suppresses myometrial contractility and local immune responses, and favours cervical closure, thereby creating conditions conducive to bacterial colonisation. Although Escherichia coli remains the predominant bacterial pathogen, the microbiological spectrum is considerably broader and may include mixed bacterial communities. The success of conservative management depends on timely diagnosis, careful clinical assessment, and appropriate patient selection. Suitable candidates are clinically stable breeding bitches without severe systemic complications, whereas conservative treatment is inappropriate in animals with sepsis, peritonitis, uterine rupture, or severe renal or hepatic dysfunction. Aglepristone represents the cornerstone of pharmacological management. By blocking progesterone receptors, it disrupts progesterone-dependent uterine mechanisms, promotes cervical opening, and facilitates evacuation of pathological uterine contents. Mifepristone provides an alternative oral antiprogestin, whereas prostaglandins promote luteolysis and myometrial contractility. Their use, however, requires careful consideration of cervical patency and the animal’s clinical status because inadequate uterine drainage may increase the risk of adverse effects and complications. Conservative management of canine pyometra therefore requires careful and differentiated patient selection rather than a uniform therapeutic approach. Antiprogestins, particularly aglepristone, have a central role in fertility-preserving pharmacological management, whereas the use of prostaglandins should be guided by the clinical condition of the animal and the potential for effective uterine drainage.
